The Credit Valley FHT - Overview
The Credit Valley FHT (CVFHT) is a group of family physicians, residents and health professionals who provide primary care services to approximately 11,000 patients in the Mississauga area. The CVFHT is associated with the Credit Valley Family Medicine Teaching Unit, and is located in the Professional Building at the Credit Valley Hospital site. It is funded by the Ministry of Health and Long-Term Care.
The CVFHT has been incorporated since May 2007 but many of our physicians have been serving the community for many years, providing care for patients from birth to old-age.
Helping You Stay Healthy
A Family Health Team is a new way of providing primary health care in Ontario. It’s a team approach that brings together family physicians with other health care providers in order to coordinate the best possible care for patients. Everyone on the team works together to meet your needs. Our focus is on keeping you and your family healthy, not just treating you when you are sick.
When you enroll as a patient with our Family Health Team, you are taking a step to ensure that you always have convenient access to regular check-ups and ongoing health care from a family physician and/or a team of other health care providers.
We are an Academic Family Health Team and therefore you may also be seen by a Resident, who is a physician specializing in Family and Community Medicine, and other students from varying healthcare backgrounds.
Building a stronger health care system
Our Family Health Team is helping to build a strong health care system.
- Helps keep the Community healthy, by giving better access to physicians, along with other health care providers.
- Takes pressure off hospitals and the health care budget by ensuring problems are diagnosed early and treated appropriately.
- Reduces the strain on hospital emergency departments by offering evening and weekend clinics.
Here at the Credit Valley Family Health Team, we strive to provide the best possible care using a interprofessional approach to meet the needs of the community.
