The Credit Valley FHT offers a number of services via referrals by doctors in the local area, as well as a number of programs promoting health and disease prevention.

Diabetes Program

People with diabetes work with a diabetes care team, which may include a family physician, diabetes nurse educator, dietitian, optometrist, social worker, chiropodist, pharmacist, and endocrinologist. Your diabetes care team is available to help you manage your diabetes and maintain your good health.

 

Lung Health Program

The Credit Valley Family Health Team has partnered with The Credit Valley Hospital and GlaxoSmithKline to provide a PRIISME ™lung health program called “Breath for Life” for patients with asthma or chronic obstructive pulmonary disease (COPD)

Nordic Pole Walking Program

We have introduced this program to help our patients gain or maintain an active lifestyle beginning with a supervised walking group.

The general goal will be to increase activity by at least 1 walk per week. Longer term goals will be to gradually increase activity as tolerated. This group will serve as the basis for information about leading a more active healthy lifestyle. This group will be provided with links in the community that suit the individual to further pursue a more active lifestyle.

Smoking Cessation Program

The Smoking Cessation program is designed to support all patients who smoke as well as patients who wish to assist someone close to them to stop smoking. It meets the needs of patients at all stages – those who do not want to currently quit smoking to those who are ready to stop. Patients will meet one on one with the pharmacist or nurse practitioner for counseling, to assess their smoking, discuss risks and to develop a plan at their own pace. Patients will be linked to community supports and receive up to date literature and resources. On-going support in person, by phone or internet are provided until your goals are met.
