Board Member
The Credit Valley Family Health Team (CVFHT) is an Academic Family Health Team affiliated with Trillium Health Partners and the University of Toronto. It is an engaging multi-disciplinary primary care team dedicated to continuous learning and excellence within primary care with a demonstrated track record of impactful quality improvement activities. The CVFHT also have a mandate to provide French language primary care services to the Francophone community. The CVFHT’s mission is to shape the future of primary care through education and high-quality collaborative care. The vision of the organization is to provide excellent primary care through partnership and collaboration at all level.
CVFHT is a not for profit organization that receives funding from the Ministry of Health and Long Term Care and provides primary health care services to over 12,000 patients in Mississauga. Families and patients are partners with their health care providers and are engaged in all aspects of their health and wellness at the CVFHT. As a board member, the patient and family advisor supports the work of the FHT and provides oversight, leadership and stewardship of the organization. While day-to-day operations are led by the Executive Director, the ED-Board relationship is a partnership, and appropriate involvement of the Board is both critical and expected.
There is no remuneration for this position.
Skills
The ideal candidates will have the following knowledge, skills and attributes:
- Commitment to the health of the North West Mississauga community and the mission and goals of the CVFHT and to live or work in the community,
- Experience on corporate or non-profit Boards of Directors with an understanding of the roles and responsibilities of a Governance Board,
- Demonstrated leadership in their profession/business,
- Relationships in the various geographic and ethno-cultural communities that our FHT serves particularly the francophone community,
- Demonstrated knowledge in financial literacy,
- Capital development and small business investment experience to support clinic redevelopment,
- Willingness to commit time and energy and participate in approximately 6 - 8 morning board meetings a year, and one half day retreat per year for a 2 year term.
Qualifications
Every director shall:
- Be eighteen (18) or more years of age;
- Not hold a paid position with the organization and not enter a contract for services with the organization;
- Not be an immediate family member of an employee of the CVFHT, including a spouse, parent, child, sibling or other person who has an economic dependence on the employee.
- Not have the status of bankrupt;
- Not be a person who has been found under the Substitute Decisions Act, 1992 or under the Mental Health Act, to be incapable of managing property;
- Not be a person who has been found to be incapable by any court in Canada or elsewhere;
