Position: Certified Diabetes Educator/ Registered Nurse

Organization: Credit Valley Family Health Team (CVFHT) - Mississauga, ON

Status: Permanent Part Time (22.5 hours per week).

Hours: Weekdays & occasional evenings (7.5 hour shifts)

(Hours are subject to change based on operational needs)

Who we are:

CVFHT is an Academic Family Health Team affiliated with Trillium Health Partners and the University of Toronto. It is an engaging inter-professional primary care team dedicated to continuous learning and excellence within primary care with a demonstrated track record of impactful quality improvement activities. It is a not for profit organization that receives funding from the Ministry of Health and Long Term Care and provides primary health care services to over 12,000 patients in Mississauga. The CVFHT also has a mandate to provide French language primary care services to the Francophone community.

About the role:

This is an exciting opportunity for a nursing professional to further advance clinical skills and broaden their knowledge base. You will be part of a dynamic multidisciplinary team involved in patient assessment and teaching. In addition, you will have the opportunity to contribute to research and education activities, which are essential components of the Program.

Day to day responsibilities:

In collaboration with the Family Health Team, you will be assess, plan, implement, provide care coordination and evaluation of clients who are referred for diabetes care and management. As part of the Diabetes Education Program (DEP), you will work with the DEP dietitian to provide diabetes care to clients of CVFHT, as well as to community clients.

· Triage referred patients with prediabetes and type 2 diabetes to determine health status, diabetes self-management skills and learning needs, diabetes management and glycemic control.

· Provides diabetes self-management education (individually or in group) to people with diabetes and their families or significant others both individually and in groups.

To thrive in this role, you’ll need:

Education/experience:

· Currently registered with the College of Nurses of Ontario.

· Certified Diabetes Educator (CDE®) designation preferred or in process.

· Experience in diabetes education, both in individual and group settings.

· Satisfactory attendance and employment record required.

· At least 2 years’ RN experience desired, preferably in a community setting

· Excellent knowledge of insulin and Oral Anti-Hyperglycemia agent (OHA) treatment based on the 2018 Clinical Practice Guidelines / 2020 Update.

Skills and abilities:

· Demonstrate strong leadership and problem solving abilities.

· Excellent time management and organisational skills.

· Excellent written and verbal communication skills.

· Ability to speak French as a second language, an asset.

· Able to establish positive and supportive relationships with patients, families and Allied Health Care Professionals.

· Computer literacy (Microsoft; EMR- Accuro, virtual platforms - OTN and Zoom).

· Strong interpersonal skills to interact effectively with stakeholders at all levels

· Foot care assessment and teaching an asset
