Primary Function or Purpose of Job or Position

Credit Valley Family Health Team (CVFHT) is an Academic Family Health Team affiliated with Trillium Health Partners and the University of Toronto. It is an engaging inter-professional primary care team dedicated to continuous learning and excellence within primary care with a demonstrated record of accomplishment of impactful quality improvement activities. It is a not-for-profit organization that receives funding from the Ministry of Health and Long-Term Care and provides primary health care services to over 15,000 patients in Mississauga. The CVFHT also has a mandate to provide French language primary care services to the Francophone community.

The Physiotherapist, under the direction of the Executive Director, will be responsible to advise patients and their families on health care including health promotion, disease, illness, and accident prevention. This position will collaborate with members of an interdisciplinary team to plan, implement, coordinate, and evaluate patient care and support in consultation with individuals and their families. This role will also respect and protect the rights of patients, including but not limited to their right to independence, autonomy, and self-determination; their right to choose and practice individual values, beliefs, religion, and culture; their right to privacy and dignity. The Physiotherapist must possess a degree or diploma in science, health sciences, or a field appropriate to the position and certifications or licenses appropriate to the position.

  • Ability to maintain positive professional boundaries without imposing personal judgments and expectations on families and patients
  • Ability to make decisions in complex situations that could be immediately dangerous to life or health where no established guidelines exist
  • Ability to effectively communicate both verbally and in writing: Superior communication, problem solving, tact and conflict resolution skills are required
  • Ability to work individually as well as part of a team
  • Demonstrated time management skills
  • High flexibility with strong interpersonal skills that allow one to work effectively in a diverse environment
  • Computer literacy
  • Proven knowledge of case management processes and procedures
  • High level of integrity and work ethic
  • Positive attitude, good judgment skills, diplomatic, team player
  • Creative, innovative and enthusiastic
